In accordance with the data protection act 1998 and access to health records act, patients may request to see their medical records.

Requests for paper copies of your medical records should be made to the Practice and may be subject to an administration charge in some circumstances.

No information will be released without the patient consent unless we are legally obliged to do so.

You may also request access to your medical records through the online access facility. Should you wish to request this please speak to a member of the reception team.
