Sickness Certification

There is no need to provide a doctors note for the first seven days of absence. Instead you should completer a form 'SC2' or self certificate. If your employer requests a doctors note before the first seven days of absence then this is considered outside of Government recommendations for sickness certification and is classed as a 'private sick note' for which there will be a charge to yourself or employer. Employers may not always be aware of the relevant guidance around sickness certification so it may be worth discussing the above with them if they are requesting a Doctors note before the seven day period has lapsed.

Fit Note

If you have been off work for more than seven days then you will need a 'fit note' or form 'Med 3' from your Doctor to certify your absence. If you have recently seen a Doctor or Nurse Practitioner at the surgery in relation to the illness causing absence, then you may not need a further appointment for the Med 3. Contact our reception team to discuss further if you require a form Med 3.

Fit For Work

'Fit For Work' is a Government program available if you have been off work for more than 4 weeks with illness, and do not have access to an Occupational Health service via your Employer. It provides free access to high quality Occupational Health advice to support a return to work in a safe and timely manner.
